Canadian trained nurses currently or previously registered as a nurse in Canada

Documentation required for Phase 1:

Identification document;

Name change documentation, if applicable;

Certificate of nursing and/or midwifery education program completion;

Transcript of nursing and/or midwifery education;​

Letter of Standing sent directly to HSCPOA from the nursing/midwifery regulatory body in the respective jurisdiction.

Documentation required for Phase 2:

Criminal Record Check (Criminal Record and Judicial Matters (CRJM) or Vulnerable Sector Check), within the past 12 months;

Headshot Photo, and;

Letter of Standing from an applicant’s other licensing/regulatory body (not nursing/midwifery related, if applicable) sent directly to HSCPOA via email.

NOTE: Applicants can complete the required documentation for Phase 2 while their eligibility assessment under HSCPOA’s Recognition of Nurses and Midwives Policy is in progress.
